The Terms:
Services start on the date of purchase with your monthly subscription renewing automatically. To opt-out and avoid auto-renewal for the following month, email info@moonbrookmedicine.com no later than 48 hours before your membership purchase date. Included services do not roll over and must be used within the monthly billing cycle. Each membership is 'as is'. Switching services is not allowed.

Please be proactive when booking your services each month as they do not roll over. You can call the office during business hours or schedule them online. If there are no available appointments that work for you, we can add you to the waitlist or you can add yourself to the waitlist on your patient portal.

If you have an insurance plan that includes acupuncture benefits, we require that you use your covered visits before enrolling in our membership program. Please note that the membership is not applicable if your insurance offers unlimited covered visits.
